This grants permission to Richard Hartman and/or Antoinette Aubert to take photos of the skater named below. The photos are for personal (hobby) use only.
In additional, separate permission is asked to possibly publish the photos on the skate fan web site catslair.com (owned and operated by Richard Hartman and Antoinette Aubert). This is separate from the permission for taking photos. No other potential use is planned or requested at this time.
Permission may be granted for the current event, the entire season, or until we recieve a cancellation request from the skater.
You must be 18 in order to grant permission. If you are under 18, a parent or guardian must sign instead.
If an email address is provided then the photos will be made available to the skaters for printing through walgreens.com. There is no fee charged for this beyond whatever fee Walgreen's charges for making photo prints.
